• Program Supplemental Application
• ACORDs (125, GL, & Umb)
• GL Exposures
• Vehicle Schedule
• Driver List (DL #, DOBs, and DOH)
• MVRs, if available
• 5 Yrs Currently Valued Auto/GL Loss Runs
• Complete Copies of Auto & GL Underlying Quotes
• Detail on any AL or GL losses greater than $50k
ELIGIBLE RISKS
• Fleets up to 50 Heavy/Extra Heavy Vehicles
• Total Fleet (including PPT, Light, & Medium Units) up to 75 Power Units
• No Restriction on Radius of Operations
• Low Hazard GL Exposures
